Goal: The goal of this program is to end homelessness in the City of Portland and Multnomah County by 2015. As efforts to end homelessness continue, city and county officials will focus on nine actions. Programs throughout the county will address moving people into Housing First, ending the practice of discharging people into homelessness from jails and hospitals, improving outreach, emphasizing permanent solutions, increasing the housing supply, creating new partnerships, improving the rent assistance system, increasing economic opportunity for homeless people and implementing new data collection technology.

Goal: The goal of the CBHC is to increase the consumption of 1% (low-fat) milk in order to prevent osteoporosis among low-income Latino mothers.

Impact: Immunization rates increased from 53.2% to 72.4% and parents reported a better understanding of the importance of timely vaccinations.

Goal: The goal of this intervention was to increase HIV preventative behavior among inner-city minority adolescents.

Impact: Students in the classroom-based intervention group had more sustained changes in HIV prevention behavior over time compared to those in the peer-based intervention groups.

Goal: The Janice Mirikitani Family, Youth and Childcare Center provides award winning childcare, after-school programs, and parenting resources to low-income and homeless families.

Goal: The goal of this program is to reduce the use of cigarettes, alcohol, and marijuana among adolescents.

Impact: Evaluations of the project showed that there was a smaller increase in students who intend to use cigarettes, alcohol, and tobacco within the upcoming months and that there were significant effects on the proportion of students reporting the use of cigarettes, alcohol, and tobacco.

Goal: Million Hearts seeks to prevent one million heart attacks and strokes in the next five years (by 2017) by coordinating cardiovascular prevention implementations in its private-public partnerships.

Goal: The goal of the MEND program is to reduce obesity levels in children by offering free healthy living programs that aim to encourage small lifestyle changes that improve health.

Impact: The MEND program was successful in reducing waist circumferences and BMI scores while increasing cardiovascular fitness, physical activity, and self esteem in children placed within the intervention group. The results of this study suggest that the MEND program is a promising intervention to combat rising child obesity rates.
